You can submit your form with any travel and accommodation invoices to your local IPTAAS office or email us at iptaas@health.nsw.gov.au.
The IPTAAS team will assess your application and may contact you for more information.
Applications must be submitted within 12 months of your hospitalisation discharge or appointment end date.
IPTAAS requires a new application form to be completed each time you visit a new specialist or specialised health service.
